Progress Monitoring Forms®

We offer progress monitoring sheet options labeled as PM-A, PM-B, or PM-C. Forms A and B are used with the Size Matters® handwriting practice program, while form C is used to track the individual goals of each student. Here’s an overview of how to use each sheet:

• Form A: The 11 steps in Form A track accuracy in letter size for upper and lower case letters individually and in words of increasing length. You can monitor mixed letter sizing with words as short as three letters or as long as ten. Use each of the seven trial boxes on the grids to determine whether a student is ready to advance. You can score for size, errors in shape, spacing, slant, directionality, omission, recall, case substitution, and letters with horizontal orientation or reversals.
• Form B: This form continues from Form A with steps 12 through 15, focusing on letter size, spacing, and copying skills for short phrases, sentences, and paragraphs. You can score for Spaghetti and Meatball spacing, Direct Line copy, Near Point, Mid Point, and Far Point.
• Form C: The final form focuses on the individual goals you’ve given each student. You can measure the amount of assistance they need, the type of prompt that works best for them, the quality of the task once completed, and the carryover of their practice sessions. You can also choose to create your own metrics for tracking success!

• Form B: Six trial boxes per step (works with SMHP®)
